IMPORTANT: The quality of the copyright application forms submitted for registration directly affects the quality of the copyright registration certificate you receive. The Copyright Office produces copyright registration certificates by means of an optical scanning system which utilizes an image scanned from the original application submitted. As a result, any copyright application you might submit which is of a poor quality will negatively affect the print quality of the copyright registration certificate you receive. Care should be given to submit the best quality application form possible.

Print the forms back-to-back using BOTH SIDES of 8.5 inch x 11 inch standard white 18 to 22lb. bond paper. (The top of page two should be directly behind the top of page one). Please do not use this, if you have a dot matrix printer. Instead, call the Copyright Office Forms Hotline at (202) 707-9100 any time day or night to request that they mail you the form(s).
